enhanced primary care

Since July 2006, Pangula Mannamurna has been attempting to provide Enhanced Primary Care (EPC) for Indigenous people in the South East Region.

The EPC approach has taken hold since the introduction of the listing of the EPC and Chronic Disease Management (CDM) items under the Medicare Benefits Schedule.

The EPC provides a framework for a multidisciplinary approach to health care through a more flexible, efficient and responsive match between care recipients’ needs and services. It consists of 28 MBS items, which include:

  • Health Assessments for Aboriginal and Torres Strait Islander people ;
  • Multidisciplinary care planning;
  • Multidisciplinary case conferencing;
  • Multidisciplinary discharge care planning and case conferencing.

People with chronic conditions and complex care needs that are being managed by their GP under an EPC care plan are also eligible for some allied health and dental services. Also, GPs have access to Medicare rebates for preparing and reviewing ‘GP only’ management plans for patients with chronic medical conditions (the CDM Items). For patients requiring multidisciplinary care, GPs can also claim from Medicare for coordinating team care planning and review services.

The purpose of the health check is to make sure that Aboriginal and Torres Strait Islander people receive primary health care matched to their needs, by encouraging early detection, diagnosis and intervention for common and treatable conditions that cause considerable morbidity and early mortality.
